Aims:	To develop an understanding of the issues surrounding transition for young people with palliative care needs, their families and professionals.
Content:	From a Northern Ireland perspective: including shortfalls, needs, problems, available services, young people’s views, evidence of ‘good practice’ and poor practice. The way forward: ACT Care Pathway, standards, children with complex needs. UK Initiatives: Brief Overview.
